Jean McCandless, MSW, LiCSW

CHAMPLAIN GERIATRIC SERVICES is the Elder Care Social Work practice of Jean McCandless, MSW, a Licensed Clinical Social Worker with national certification in geriatric case management. Jean is committed to helping elders stay in their own homes as long as possible, and assisting families to find appropriate alternative living situations when necessary.

Jean provides elders and their adult children with respectful and well-coordinated services, including: assessment; case management; counseling; and caregiver supervision. Much of her work occurs in people’s homes.

Jean specializes in working with elders whose family members live at a distance. In addition to arranging needed services, she often provides ongoing supervision through regular home visits and contact with physicians and other service providers. Such oversight and care coordination is available on either an hourly or retainer payment basis.

In addition to her 20 years as a Geriatric Social Worker, Jean is also a Vermont Qualified Development Disabilities Professional (QDDP). Her human services experience includes extensive work assisting people with physical and cognitive disabilities and Traumatic Brain Injuries.
