CHAMPLAIN GERIATRIC SERVICES is the Elder Care Social Work practice of Jean McCandless, MSW, a Licensed Clinical Social Worker with national certification in geriatric case management. Jean is committed to helping people stay in their homes, and to providing elders and their adult children with respectful and well-coordinated services. These include: assessment, case management, counseling and caregiver supervision. Much of her work occurs in people’s homes. Jean is also a Vermont Qualified Development Disabilities Professional (QDDP). In addition to her 20 years as a Geriatric Social Worker, Jean’s human services experience includes extensive work assisting people with physical and cognitive disabilities and Traumatic Brain Injuries.
